  • Congratulations to Nicholas Gonzalez, Partners' Community Linkages Specialist, for receiving Employee of the Month for July 2024! Nicholas joined the agency in May 2022 as a care coordinator for the ECM Department, and in September 2023, joined the Communications team as the Community Linkages Specialist. His role currently focuses on community relations and outreach for Partners’ programs, with a focus on supporting the Multipurpose Senior Services Program (MSSP) by increasing referrals and enrollments. Within the past year, Nicholas has conducted forty in-person and virtual MSSP presentations to CBOs, health plans, hospitals, senior centers, and other senior/health care facilities. These presentations resulted in an 85% increase in MSSP website referrals compared to the previous year and over fifty new community referral sources. Most notable presentations conducted include Blue Shield CA social workers, Harbor UCLA team of direct care providers, USC health consultants, and Anthem Blue Cross’s care coordination team. Nicholas possesses strong public speaking and interpersonal skills, and his ability to develop long term relationships through effective community engagement makes him an integral part of the Partners’ team. Nicholas, we are constantly amazed by your exceptional performance. Congratulations!

  • Congratulations to Lavada Deel for receiving Employee of the Month for June 2024. Lavada “Vada” Deel joined the Home and Community Based Alternatives (HCBA) Waiver Program in July 2022 as a Case Manager. She is a graduate of California State University, Los Angeles' MSW program and actively serves as a preceptor for HCBA interns. Before joining Partners in Care Foundation, Vada worked in Hospice and Palliative Care, providing emotional support and end-of-life coping strategies to patients and their families. She also served as a Care Coordinator at Visiting Angels Home Care Agency, assisting individuals and families in managing the effects of aging, diseases, and terminal illnesses. Moreover, she undertook leadership responsibilities by training and mentoring staff members. Vada's wealth of prior experiences, resource knowledge, and her remarkable ability to connect with clients make her an asset to the HCBA program. She adeptly handles a demanding caseload, yet continually aids colleagues when necessary. During her tenure at Partners, Vada has conducted comprehensive training sessions, shadowing opportunities, and maintained a positive, approachable demeanor. Her skill in defusing difficult situations with clients, securing elusive signatures, and overcoming communication barriers underscores her persistence, dedication, and effective communication abilities. Vada actively enhances her skills by engaging in training sessions and showing initiative during DHCS calls. Her confidence, organizational prowess, and efficient assessment turnover rate are truly commendable. We are delighted to have Vada as a part of our team.

    Partners' recognizes today, June 19th as Juneteenth. Juneteenth, also known as “Freedom Day” and “Emancipation Day,” is a significant day in American history. Juneteenth marks the day when federal troops arrived in Galveston, Texas in 1865 to take control of the state and ensure that all enslaved people be freed. Going forward, the country now recognizes Juneteenth each year to reflect on our history and commit to doing better. The legacy of Juneteenth illustrates the power of African Americans’ perseverance, strength, resilience, and refusal to give up hope, even in the toughest of times. However, June 19, 1865, did not mark the end of their journey towards achieving freedom, equality, and justice. It was only the beginning. Let us recognize the accomplishments of those who have fought long and hard for civil rights and social justice and acknowledge the work that we must continue to do to address institutional racism and systemic inequality.
