When performing Root Cause Analysis, identifying a single chain of events does not always provide a complete answer for why the event occurred. Many times, we must explore multiple paths from causes to the event. This training will discuss 5-Why analysis with multiple paths and explore the use of Causal Factor Maps and Fault Tree Analysis. We will discuss the use of AND and OR gate logic in Fault Trees and look at examples of events that were the result of multiple contributing and root causes. ARINC858P1-1: 858P1-1 Internet Protocol Suite (IPS) for Aeronautical Safety Services, Part 1, Technical Requirements ARINC 858 Part 1 defines the airborne data communication network infrastructure for aviation safety services using the Internet Protocol Suite (IPS). ARINC 858 builds upon ICAO Doc 9896, Manual on the Aeronautical Telecommunication Network (ATN) using Internet Protocol Suite (IPS) Standards and Protocol.
