Position Summary

The member will serve to support CAN Community Health's mission "to inspire and contribute to the health and well-being of those affected by HIV, Hepatitis C, and other sexually transmitted diseases by providing the best care through outreach, integrated clinical practice, advocacy, education, and research." CAN Community Health provides prevention services free of charge to the community by providing rapid HIV and rapid Hepatitis C testing in the clinic and at community outreach events, providing free testing for sexually transmitted diseases (syphilis, chlamydia, & gonorrhea). and by providing access to PrEP (Pre-exposure Prophylaxis - the medication that prevents HIV) to anyone regardless of health insurance status. CAN Community Health also provides linkage to other community resources such as assistance with access to mental health, substance use disorder treatment, and other community resources.

Major Duties and Responsibilities

  • Providing outreach to agencies to build awareness
  • Developing and providing health education for the community
  • Providing education to clients and referring them to providers
  • Providing health education to at-risk population
  • Attending trainings and/or community events
  • Engaging in outreach and promoting health education topics and healthcare services.
  • Collaborating with pre-existing patients and partner organizations.
  • Identifying hosts and sponsors in various communities to form new partnerships.
  • Creating healthcare and wellness educational materials.
  • Facilitating and conducting healthcare and wellness trainings and classes.
  • Providing health counseling and coaching.
  • Keeping and organizing documentation and records.
  • Developing community relationships.
  • Providing rapid HIV testing, rapid Hepatitis C testing, and testing for other STIs.

  • Effective customer service, oral and written language skills, community outreach and engagement, HIV testing experience, counseling experience, data management/entry, and previous experience with office tools such as landline phones and corporate printers/fax machines.
  • Flexibility when interacting with clients and individuals with various backgrounds and life experiences.

Knowledge Required for the Position

  • Knowledge of AmeriCorps/Health Corps member requirements
  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

The member will be provided daily support with any technical assistance needed with Microsoft Office or other software platforms after the initial training period. This support is provided by other team members and CAN Community Health's IT Department. A comprehensive understanding of the requirements, responsibilities, and expectations of the member as an NHC presence is essential for the member to successfully perform the duties of the position. Member will need to heavily rely on Microsoft Office software including Teams, Word, Excel, and Outlook as these platforms are used extensively in their daily workflow. Basic Microsoft Excel skills will be useful when recording and managing data. Teams and Microsoft are used as the primary communication platforms in the company.

Supervision

  • Member uses initiative in carrying out recurring assignments following set procedures, independently.
  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required.
  • The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.

All NHC members will complete a Pre-Service Orientation provided by the NHC Program that provides key information about NHC and AmeriCorps policies, procedures, and requirements, as well as foundational skills development and team-building opportunities. NHC members will receive a Training Plan from their Host Site that details training that will be provided specific to the member’s Host Site service position

Review

  • The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
  • The member independently plans and carries out the projects and selects the approaches and methods to be used in solving problems.
  • Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.

All NHC members are given a written performance review by their Host Site supervisor and NHC Program Director at the mid-term and end of their term of service. Performance reviews are based on the member’s performance at their Host Site and their participation in NHC responsibilities such as member trainings, committees, and group service projects.

If a member disagrees with any aspect of their performance review, they can appeal to their Program Director according to the grievance procedure outlined in the NHC Member Handbook.

NHC members will be evaluated according to the following criteria:

  • Whether the member has satisfactorily completed service assignments, tasks and/or projects;
  • Whether the member has met any other NHC expectations which have been clearly communicated orally and/or in writing throughout the service term;
  • The member’s performance in competency areas identified by NHC;
  • Whether the member has completed or is on track to complete the required number of hours outlined in their member contract.

Guidelines

  • Written and oral guides provide specific instructions for doing service.
  • Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards and files of previous projects.
  • Member must use considerable judgment in adapting current or developing new guidance

Members are trained for HIPAA compliance, outreach guidelines, clinical guidelines, and are provided with multiple training modules. Written and oral communication skills are essential in this position to follow guidelines, policies, and procedures put in place to provide optimal services to the community. Critical thinking skills may be needed in unique circumstances where there are no written guidelines in place.

Complexity

  • Member has little or no choice about how to perform the service position.
  • The member must develop new techniques, methods, or approaches to complete service position assignments

For activities such as the provision of testing, member will have to follow strict instructions and policies to accomplish this task. For activities such as the development of education plans, member will have leeway but will need to be able to research and incorporate evidence-based information.

  • The ability to lift at least 20lbs, the ability to sit, stand, and walk for 9 hours a day to access clients for testing services and provide outreach throughout the community.
  • Host site will reimburse service members for mileage to and from outreach events that are not hosted at the host site location (community outreach events not held at the clinic location.)

Criminal History Check Requirements Beyond NHC Standard Checks

Background checks notifies host site of any violations in the last 7 years. Supervisor reviews all candidates and consider them for employment. With felony convictions, CAN Community Health’s 3rd party investigation firm asks us if they want to send out a rejection letter. Offenses including theft, assault/battery and anything to do with children are heavily scrutinized.
