SUMMARY We are seeking a dynamic and compassionate Health Coach and Care Navigator to join our team. In this role, you will work closely with the club’s Physicians, Nurse Operations Leader, facility clinicians, and specialists. You will play a pivotal role in supporting clients on their journey to health and wellness by providing guidance, education, and assistance in navigating their healthcare plans with a focus on functional and integrative medicine. The ideal candidate will have a strong health coaching background in functional medicine, as well as primary care, lifestyle, and integrative medicine. In addition to a passion for promoting holistic health behaviors and interventions using excellent communication skills and motivational interviewing.

Essential Functions

Client Guidance and Education:

• Monitor goals and act as a supportive resource for clients, helping them understand and navigate their treatment plans, in the context of functional medicine with a focus on fitness.
• Provide evidence-based education from a functional medicine perspective on lifestyle factors including nutrition, fitness, stress management, and other relevant topics to empower clients in taking control of their health.
• Developing learning and educational content based on coaching principles and collaboration with medical professionals for use with clients in care plan development and coaching.
  
  

Individualized Health Coaching:

• Conduct independent in-person and virtual one-on-one coaching sessions in goals collaborate with clients to set goals and personalize healthcare teams’ plans for achieving optimal health.
• Collaborate with healthcare providers to integrate functional medicine to lifestyle interventions into clients' treatment plans.
• Engage in ongoing medical education to remain evidence-based and current to support functional, integrative, and lifestyle related care plans for client care, in collaboration with health professionals.
• Incorporate biometric data, wearable information, and other objective data into client care plans.
  
  

Support Group Coordination:

• Assist in the research and creation of content for online and group education.
• Participate in development and deliver group education sessions, to both small and large groups.
• Organize and lead support groups to foster a sense of community and mutual support among clients with similar health challenges.
• Participate in community outreach projects and support programing for organization.
  
  

Care Coordination:

• Participate within the interdisciplinary care team by working closely with various health care professionals including physicians, nurse practitioners, physical therapists, eastern medicine, fitness trainers, behavioral health, and registered dietitians to coordinate seamless client care.
• Present specific client case information at the daily or weekly huddles as a collaborator within the larger care team.
• Assist in implementing care plans that align with the Love.Life holistic approach and client’s individual health values and needs.
• Support clients with coordinating internal and external referrals and scheduling services across the wellness and fitness center and interdisciplinary teams.
  
  

Client Advocacy:

• Advocate for clients' needs and preferences within the healthcare system.
• Assist clients in navigating insurance coverage, referrals, and other administrative aspects of their healthcare.
• Support clients as a Love.Life care navigator and resource of offerings and services.
• Support clients’ needs with Love.Life’s application and technology to support integration and understanding of precision health data.
  
  

Job Knowledge, Skills, Abilities

This Health Coach and Care Navigator must possess the following knowledge, skills, and abilities to demonstrate they can perform the essential functions of the job, along with the physical requirements, with or without reasonable accommodation.

• Strong understanding of primary care, functional, integrative, fitness education, and lifestyle medicine interventions and care planning.
• Previous experience in health coaching is preferred.
• Excellent communication and interpersonal skills.
• Empathetic and compassionate approach to client care.
• Strong familiarity with Electronic Health Records systems.
  
  

Experience/Education Required

• Current certification as health coaching or accreditation eligible, at time of hire.
• Training in Functional Medicine Health Coaching, preferred.
• Bachelor's degree in Nutrition, Nursing, or a related field, preferred.
• Current BLS with AED certificate from the American Heart Association or similar organization.
