Summary

Health coaching can potentially address several of the barriers to evidence based care including: limited access to care, poor coordination of care, distrust of and lack of engagement with the healthcare system, missed appointments, inadequate adherence to treatment, and poor patient-provider communication.

The iHealth Certified Health Coach will work closely with a Dietitian and Clinical Assistant in a fast-paced, technology-driven environment to provide patients with high-quality care regarding Hypertension, Hypotension, Diabetes, Pre-Diabetes, and Obesity. This position will be part of a wider-scope endeavor to test the feasibility and potential impacts of a health coach-facilitated and coordinated care plan with lifestyle goal setting as a fundamental cornerstone.

The individual will receive extensive training at the beginning of employment to gain the knowledge and skills needed to work with patients in an individualized manner. The Health Coach will review patient records, gather relevant patient data, meet with referred and enrolled patients, and provide basic health education regarding diet and vitals to support the implementation of the recommendations accepted by the primary care clinician. The Health coach will consult with the Dietitian, Clinical Assistant, and contracted Primary Care Provider regularly, and act as a representative of the care team when following up with patients.

Job Responsibilities

  • Inspire patients seeking to increase healthy lifestyle behaviors in areas such as weight management, eating better, getting more active, managing stress and prevention/management of chronic health concerns like diabetes or hypertension.
  • Review patient records and gather pertinent health metrics, including blood pressure, glucose levels, weight trends, and food diary insights, while facilitating and reviewing routine screening questionnaires with patients
  • Offer virtual coaching sessions to assist patients in setting, adjusting, and achieving their medical and lifestyle goals with foundational health education on Hypertension (HTN), Hyperlipidemia, Diabetes Mellitus (DM), Pre-Diabetes (pre-DM), and Obesity
  • Use coaching skills and competencies within a Motivational Interviewing framework to help members set concrete, SMART health goals
  • Ensure guidance aligns with iHealth’s Care Standards, in conjunction with AHA and ADA recommendations, and aligns with any recommendations of our specialists also contributing to this patient’s care.
  • Collaborate with other providers when indicated in a coordinated care team to ensure the appropriate level of care and best-in-class member experience
  • Assist the Clinical Assistant in patient onboarding, support, and engagement to enhance monitoring frequency and overall team interaction
  • Escalate cases requiring individualized Medical Nutrition Interventions to Registered Dietitians and other specialists when clinical input is desired
  • Communicate on behalf of the team to engage professionally with clinicians, staff, and patients and maintain positive relationships
  • Follow department policies and procedures as detailed in company manual documents when charting on visits, during monthly chat and virtual check-ins, and daily patient and clinical team communication
  • Conduct patient outreach calls following provided scripts to schedule appointments, provide follow-up care information, and promote health education
  • Visit assigned clinic locations on assigned, pre-scheduled dates as needed to provide face-to-face consultations for at risk populations and to assist with onboarding as necessary.
  • Aid in administrative tasks such as data entry and scheduling when necessary
  • Work within iHealth’s product suite to maintain strict confidentiality at all times, including diligently following HIPAA guidelines and processes to ensure protection of Protected Health Information (PHI)
  • Stay current on all training requirements and communications

Qualifications:

  • Degree in health-related field required (such as nutrition, public health, exercise physiology, psychology, kinesiology, health science, or other related discipline) OR an AA with two (2) years of health and wellness-related work experience and/or training
  • Health coaching certification from an accredited, National Board for Health & Wellness Coaching (NBHWC) approved health coaching program (i.e., Wellcoaches, Duke Integrative)
  • Required to pass iHealth’s internal clinical condition care readiness assessments within probation period
  • High level of familiarity with technology such as GSuite (GCalendar, GSheets, GDocs, etc.), Slack and other basic software applications
  • Experience coaching members one-on-one with health goals after receiving an approved health coach certification is a plus!
  • Bilingual proficiency in English and Spanish is a plus! Bilingual Mandarin, Cantonese, or Vietnamese is also advantageous.
  • Experience in direct patient care procedures, including checking in patients, obtaining patient histories, taking vital signs, preparing charts, and assisting with medical examinations
  • Organized, tech-savvy, and highly driven with the knowledge, skills, and abilities to guide individuals in improving their diet and lifestyle
  • Independent and self-motivated with the experience of working with and appreciating a culturally diverse patient and staff population
