In-Home Support (IHS) Specialist

Job Summary

Oak Tree Support Services is hiring for part- time Independent living specialist (ILS) to provide clients with training and resources needed to develop the skills necessary for independent living. The goal is to maximize client independence in the community. ILS Specialists will work with people ranging across many disabilities, and many age ranges. Oak Tree Support Services believes in change, and as our IHS specialist you will work with our individuals to customize a person-centered plan to help them in their everyday lives.

Supervisory Responsibilities

Duties / Responsibilities:

The ILS specialist coaches and role-models activities of daily living including, but not limited to:
  • Help client build skills to navigate their communities and connect with resources for reaching client-driven goals
  • Transports people served to medical appointments, grocery shopping, local county, etc.

  • Maintain routine schedule with each client.
  • Report all appointment cancelations to Supervisor via email.
  • Participate in purposeful activities with residents.
  • Submit all documentation of work completed and in the information management system (Naurnia) or to Staffing Coordinator as applicable before ending any shifts with a client
  • Communicate clearly to the client and division leadership on a regular basis
  • Attend quarterly virtual meetings and required trainings
  • Perform other duties as assigned by Supervisor.
Required Skills/Abilities
  • Maintain professionalism and confidentiality for all clients.
  • Excellent verbal and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills with a proven ability to meet deadlines.
Education and Experience:
  • High school diploma or equivalent.
  • Acceptable Driving History
  • Valid driver’s license and insurance
  • Must be willing to work a flexible schedule
  • Ability to pass DHS background check
  • Direct Support: 1 year (preferred)
  • ILS/IHS: 1 year (preferred)
Pay: $17 per hour

Earn mileage based on the IRS recommended mileage rates while providing services for a maximum of 6 miles per person served per shift worked.
