Open Access Team Mental Health Case Manager

If you’re searching for something more than just a job, something akin to a calling, then consider the challenge and opportunity of being a member of Johnson County Government!  Johnson County Mental Health is looking for qualified candidates to fill the position of Access Team Case Manager. The Access Team utilizes a highly structured team model to triage all prospective new and returning clients to the agency, respond to emergency calls and walk-ins, complete "open access" intakes and hospitalization screenings during daytime hours, and collaborates with internal treatment teams and community partners to link individuals to the right treatment, services and supports. Case Managers on this team may provide coverage at alternate Johnson County Mental Health Center locations as well as in the community. First review of applciations will be July 15.

 

Job duties includes:

  • Through a structured triage process, obtains information regarding individuals' background and applicable circumstances to identify needs and/or next steps in determining service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders.
  • Assesses current risk of individuals walking in or referred to Open Access crisis clinic; completes safety plans or connects individuals with higher level of care as indicated.
  • Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to coordinate appropriate services; encourages the establishment and maintenance of support systems.
  • Coordinates follow up outreach to designated clients to promote and encourage engagement in identified treatment services.
  • Completes and submits documentation regarding services provided per agency policy.

 

Minimum Job Requirements:

Bachelor’s degree in Human Services or related field and a valid driver’s license with acceptable driving record required.

 

Johnson County Government requires reference/background screening for all positions.  Specified criteria may vary by Department/Agency.

 

Preferred Job Requirements:

One year of related work experience is preferred.

 

Education/Experience Substitutions:

Experience may be substituted for degree. Education may be substituted for experience.

In some instances, statutory or regulatory requirements may prohibit substitution of education and/or experience. 

 

Physical, Environmental, and Special Working Conditions:

All County employees may be called upon to assist other departments in a declared emergency situation.
