Objective: To observe the clinical character of the diabetic patients with complicated acute myocardial infarction and to evaluate the earlyterm effect of captopril on these patients.
Methods: In a multi-centre, randomized, double -blind and placebo-controlled clinical trial of ACEI for AMI in China. In a total of 14 666 AMI patients 1 376 had diabetes mellitus. The 1 376 diabetic patients with complicated AMI were randomized into captopril (669) and placebo (707). All the patients were treated with captopril (12.5mg three times daily) or placebo for four weeks.
Results: The results of the entire study showed that the four-week mortality of AMI showed significant difference between patients with and without diabetes (14.75% vs 8.89%, P < 0.0001). The four-week mortality of AMI in patients with diabetes showed no significant difference between those treated with captopril and placebo (14.95% vs. 14.57% P > 0.05). Rates of incidence of main complications within four weeks also showed no significant difference between the two groups.
Conclusion: After AMI, for the patients with diabetes, treatment with captopril not show a beneficial effect on the early-term prognosis.
