An accurate representation of a common inferior pulmonary vein (CIPV) during three-dimensional mapping is difficult, which may lead to concerns regarding the safety of ablation. A preablation computed tomography scan revealed a CIPV with a large common ostium in close proximity to the esophagus. A detailed representation of the CIPV was possible using the new CARTO 3TM (Biosense Webster Inc., Diamond Bar, CA, USA). This allowed for safe and efficient en bloc isolation of the CIPV with no applications required near the esophagus.
