Graphnet Health is a UK supplier of integrated care record and population health software to the NHS, social and care services. It specialises in shared care records and claims to be "the UK’s leading supplier of shared care record software to the NHS and care services"[1].

The System C & Graphnet Care Alliance

Graphnet is part of the System C & Graphnet Care Alliance. The Care Alliance brings together four providers – System C, Graphnet, Liquidlogic, and CareFlow Medicines Management (formerly WellSky International) – to deliver integrated IT solutions to the UK health and social care market.

The Care Alliance employs more than 900 people and is dedicated to helping the NHS and social care providers meet the agenda of the NHS Long Term Plan - transforming services to meet existing and unmet demands, using technology and data to target care where it is most needed and provide joined up patient-centred services, and improving outcomes.

Across the Care Alliance, there are over 350,000 users of its software:

  • 28 Trusts use System C's CareFlow Electronic Patient Record system
  • Over 40 Trusts are using System C's CareFlow Vitals e-observations system
  • 90 local authorities with social service responsibilities use the Liquidlogic software
  • Medical records for more than 6 million children are managed using the CarePlus Child Health software
  • Over 20m patient records are held in the CareCentric integrated care record to share health and social care information and manage care pathways across disciplines and organisational boundaries.

History

Graphnet was founded in 1994. Brian Waters has been the Chief Executive at Graphnet since 2015.

Graphnet has been awarded several contracts, which includes providing solutions to Integrated Care Systems (ICSs). In January 2021, Swindon and Wiltshire signed a contract for a new integrated care record. The programme, using the CareCentric shared record software, programme will bring together health and care records from 94 general practices.

In April 2021, the Greater Manchester Care Record (GMCR), which is powered by Graphnet software was awarded HealthTech Project of the Year at the 2021 Digital City Awards.

In May 2021, Graphnet was awarded the contract to join up care for individuals in the Shropshire, Telford and Wrekin ICS with a new shared care record and population health analytics platform.

In 2021, Graphnet was awarded outstanding Best Companies accreditation for workplace engagement.

Graphnet’s integrated record software holds data on over 20 million citizens.

Graphnet’s health and social care solutions allow for the integration of services across whole care communities. This is achieved by linking information from multiple systems, integrating workflows between care settings, supporting multi-provider reporting & analysis and engaging patients to self-care.

The software company’s solutions use technologies - including mobile, wearables, instant messaging, the cloud and machine learning - to improve services and the way care professionals and patients interact together.
