Notes
- ^ a b Originally two chapters, the Case Tech chapter was at the Case Institute of Technology while the Western Reserve chapter, formed in 1932, was formed at Western Reserve University. The two chapters consolidated at the merger of the schools to form the Case Western Reserve University Chapter, keeping the 1904 date of formation.
- ^ The District of Columbia chapter is an area chapter serving members in the DC, MD and VA metro area who work in government, or the private or educational sectors.
- ^ The Mayo Foundation chapter serves the Mayo Foundation Community, researchers and specialists connected to the Mayo Clinic.
- ^ This chapter, not listed among active Illinois groups, was named Illinois, Chicago Medicine in Baird's 20th. Was this an 'area' chapter, designated to support the Chicago Medical Society?
- ^ This chapter originated in 1943 at the Brooklyn-based New York Technical Institute, and was called the Brooklyn Tech chapter. After school mergers it is now located at Old Westbury on Long Island and called the Polytechnic/SUNY Old Westbury chapter.
- ^ The Radcliffe chapter appears to have consolidated into the older Harvard chapter. It is not listed on the national website under the Radcliffe name.
- ^ Originally chartered as the Oregon Medical and Dental area chapter.
