I just cleaned up the section on Israel, which apparently was a translation (by a non-native English speaker) from the Hebrew documentation cited on the Knesset's website. There are still a couple of unclear points, such as that of the father's name being provided by the mother as long as the child is a minor. Ideally this section should be backed up with English-language citations, but failing that I will try to clarify further some of the statements, based on the original sources in Hebrew.
