Its easy to deliver a lecture and have a one-sided conversation but allowing Indvidual to put their thoughts to the table helps in making comfortable in this process as well as help them retrospect the situation as a third person.

In my opinion, following up helps bringing it all to a full circle and closure; if the person needs regular enforcement, help them to create a step-by-step plan. If mentorship is needed, appoint a mentor in the team. Periodic check-ins helps to completely eradicate the problem as well as demonstrate level of investment by the leader.
