    💡 Remote patient monitoring → Remote patient CARE In a new article in the Journal of Cardiac Failure Drs. Marat Fudim, David I. Feldman and Ahmed Sayed discuss how the future of digital health lies in moving beyond just monitoring to a more holistic approach. Imagine dedicated external healthcare teams providing continuous patient management, not just tracking metrics. This shift from symptom tracking to proactive care ensures that patients, particularly those with chronic conditions, receive the attention they need over time. To make this approach scalable and accessible to a broader population, centralized entities such as Cadence will be essential. This is where digital health can truly revolutionize care—by making it sustainable for all. Dr. Fudim adds, “crucially, digital health technologies isn't about replacing in-person care. Instead, it complements traditional care by helping physicians focus on the most relevant clinical data over time, ensuring better patient outcomes. This approach is especially promising for those in rural or underserved areas, where access to traditional healthcare models can be limited.     Today, we are thrilled to announce a new partnership with BayCare Health System, West Central Florida’s leading not-for-profit health care system, to provide proactive and preventive health care to seniors with chronic conditions throughout Florida’s Tampa Bay and West Central regions. The program will combine expert clinical teams with a robust technology platform to implement a remote patient care program outside of the clinic walls.     The Cadence Care Team is here to serve as your personal health coach and assistant. Today’s #MissionMonday spotlights a heart failure patient who received a home nurse referral from their primary care provider. After a few visits, they determined they no longer needed those services because they had Cadence. The home nurse simply checks their blood pressure, which is something the patient can do on their own now through Cadence. While the home nurse is only with them for a short time, one day per week, the fully-staffed Cadence Care Team checks in with patients on regular intervals to ensure they’re feeling well and staying on track with their care. The patient loves speaking to a nurse practitioner every month who has access to their records and history and shared that whenever their vitals are out of range, someone from Cadence calls to check in. This kind of complete care is what patients deserve and should expect and we are happy to be working to deliver it every day.

    In today’s #MissionMonday, we spotlight the story of a Cadence patient with hypertension who, after being enrolled for only six months in the program, reached their weight goal, losing 30 lbs with the reduction of alcohol and dietary changes recommended by their Cadence CN. Their 30-day blood pressure average is also down from 193/94 to 119/80! The patient is working on adding exercise into their routine to lose the last five pounds and could not be happier with their progress. The Cadence Care Team is made up of medical directors, nurse practitioners, registered nurses, and medical assistants. This year patients have requested more guidance to achieve their health goals and in response, we have expanded our team to include certified health coaches specializing in diet and lifestyle wellness.     This week Cadence passed a significant milestone in that we now serve 🌟 25,000 🌟 patients across the country. Even more noteworthy is that 60% of these patients are located in rural or underserved areas. Here’s a few reasons why: 🌉 Bridging the gap: Access to quality healthcare remains a challenge in rural and underserved areas. By extending care into these communities, Cadence is making strides toward equitable healthcare access for all. 🏥 Improving health outcomes: Patients in underserved regions often face higher health risks due to limited access to medical resources. Our high-quality remote patient care program ensures they receive timely care, which is crucial for managing chronic conditions and preventing hospitalizations. 💪 Empowering patients: By providing easy-to-use cell enabled devices coupled with our 24-7 clinical Care Team, we empower patients to take charge of their health, regardless of their zip code. This autonomy leads to better health management, more independence, and improved quality of life. 👩⚕️ Supporting healthcare providers: Rural healthcare providers often face significant workload pressures.     In today’s #MissionMonday, a type 2 diabetes patient shares that Cadence is their first call because our Care Team calms them down and takes the time to educate them. On this occasion, the patient’s blood glucose readings were much higher than their baseline readings. The Cadence Alert RN educated the patient on ways to help bring down the blood glucose readings and dietary considerations. The patient has had diabetes for 20 years and believes their improved health and peace of mind are a result of working with Cadence. We are encouraged by affirmations like these and continue to hone our patient experience to deliver the best care, education and support we can to our patients every day.

    It’s #MissionMonday! – Our new weekly spotlight on the life changing care Cadence’s clinicians and technology make possible every each day. These stories are why we do what we do and why building a healthcare infrastructure that allows for high-quality remote patient care is so important. This week, we spotlight a care experience had by a newly-enrolled patient: In a four day span, a patient enrolled in Cadence and began taking readings, all of which were out of range. The patient received back-to-back calls from the Cadence Alert RN team to review any potential symptoms, and then confirmed they would be seeing their primary physician in the morning. After Cadence readings showed that their heart rate was fluctuating between 120-180 throughout the day, the patient’s medications were adjusted, and they were put on a heart monitor for 72 hours for additional monitoring and to ensure they were alright. Cadence is focused on providing an extra set of eyes and peace of mind for the country’s most vulnerable patients.     Patients engage daily with Cadence, taking their vitals every morning before starting their days, and meeting with our Care Team on regular intervals between their appointments with their doctors. What we've learned over the past three years is that patients who are this engaged in their health -- day in and day out! -- want even more guidance and motivation to reach their goals. Today we are proud to launch a new Education Center designed to inform and support patients in managing their health outside of visits with the Care Team. https://lnkd.in/eX3qesJ9 Updated weekly with articles from our medical team, the Education Center will cover topics such as managing chronic conditions, nutritional guidance, exercise tips, patient stories, and caretaker support. Our clinical, product, and patient success teams are committed to continuously enhancing the patient experience, and we are excited to take this next step forward toward delivering better all around care.
