In-home services have been shown to address social determinants of health and health equity. But finding capacity and arranging services for members remains a problem. “It is always difficult to find the right provider at the right time to meet the specific needs of members,” said Travis Woyner from Dina. “Those challenges increase as members expect more in-home services from personal care, to home health, to hospice and palliative, and the expanding non-medical pool of benefits. In all of these services, staffing shortages persist, and rule changes like the Medicaid Access Rule and #CMS’s continued push towards streamlined access to care and benefits in Medicare Advantage add another layer of complexity.” As health plans adjust to a difficult landscape, Woyner answers 4 questions about improving efficiency, reviewing network adequacy, and managing out-of-network expenses. The “80-20” provision has raised concerns about access to home care. Home care agencies feel the 80/20 rule will force many companies to close their doors and go out of business, further worsening the access to home care for Medicaid members. Centers for Medicare & Medicaid Services believes this is a way to bring more caregivers into the profession to meet the demands of a growing Medicaid segment that needs home care. Says Sherman Sanchez: “One of the many motivators for passing the rule is the potential to delay progression to long-term care facilities by providing more care in the home and reducing total spend. We believe technology has a role to play here to help create efficiencies and grow capacity.” There is encouraging data that show that a Medicaid member stays in their home 21 months longer versus moving into a nursing home when the appropriate home-based services and interventions are activated. #homehealthcare #medicaid #cms #longtermcare #healthplans
