For forty years, Horizon Health Care, Inc. has been providing personalized, affordable, high-quality medical, dental and mental health care through a rural, community-based network in South Dakota. Horizon is a Federally Qualified Health Center (FQHC) that serves the medical, dental and mental health care needs of South Dakotans in rural, medically underserved areas through 105,000 patient visits annually throughout 32 community health centers in: Aberdeen, Alcester, Bison, Bryant, DeSmet, Eagle Butte, Elk Point, Faith, Fort Thompson, Howard, Huron, Isabel, La Plant, Lake Preston, Martin, McIntosh, Mission, Plankinton, Wessington Springs, White River, Woonsocket, and Yankton. Meet Saw Joseph Naykaw Kankanyaw, a 5-year-old boy whose infectious smile and dance moves hide a courageous battle against leukemia. Joseph's journey began in Myanmar, where his family faced incredible challenges to secure the care he needed. Despite political unrest and limited access to treatment, they never gave up hope. In early 2022, they made their way to the United States, where their path crossed with Horizon Health. At Horizon, we believe in treating everyone, regardless of their ability to pay. With the help of our compassionate team and the support of the local community, Joseph connected with life-saving care. Today, Joseph is nearing the end of his treatments and looking forward to starting school this fall. His mother, Naw Hailda Kanyaw Paw, is grateful for the care they received: "We really appreciate it. We thank you all." Joseph's story is a testament to the power of community and the mission that drives us every day at Horizon. Together, we make a difference.
