Did you catch Top Box Foods on CBS 2 Chicago this week? Thank you to Jackie Kostek and CBS for highlighting our USDA GusNIP Produce Prescription Program. This two year project with Medical Home Network and Alivio Medical Center is designed to determine if providing home-delivered fruits and vegetables, in tandem with care coordination and health coaching, can improve the health and food security of hypertensive Medicaid patients while lowering care costs.
