On August 14, 2024, the World Health Organization declared a global public health emergency related to the ongoing outbreak of #mpox in the Democratic Republic of the Congo and transmission to neighboring countries. The answer to yesterday’s #ClinicalChallenge question is: False. The American College of Obstetricians and Gynecologists criteria would make a presumptive diagnosis of intra-amniotic infection (e.g., chorioamnionitis) in patients with isolated fever ≥39.0°C (102.2°F) without another clear source because they are at high risk of an adverse clinical infectious outcome. However, lack of fever should not exclude the possibility in ill-appearing patients. In a study of maternal deaths due to sepsis, 18 percent of the patients had an intrauterine infection on postmortem examination (chorioamnionitis or endometritis, or unspecified diagnosis), 73 percent were afebrile on presentation, and 25 percent did not develop a fever. The FDA has approved and granted emergency use authorization for updated Moderna and Pfizer-BioNTech #COVID-19 vaccines (2024-2025 formula) to include a monovalent component that corresponds to the Omicron variant KP.2 strain of SARS-CoV-2. The mRNA COVID-19 vaccines have been updated with this formula to more closely target circulating variants and provide better protection against serious consequences of COVID-19, including hospitalization and death. The updated mRNA COVID-19 vaccines include Comirnaty and Spikevax (approved for those 12 years and older) and the Moderna COVID-19 vaccine and Pfizer-BioNTech COVID-19 vaccine, which are authorized for emergency use in individuals 6 months through 11 years of age. A large trial and meta-analysis (18 trials) in patients with sepsis/septic shock shows higher clinical cure rates with continuous versus intermittent infusion of beta-lactam antibiotics. In the meta-analysis, continuous infusion also improved all-cause 90-day mortality.  These studies suggest continuous infusions of beta-lactam antibiotics are equally effective and possibly more effective than traditional intermittent infusions in critically ill patients with #sepsis and septic shock.
