This study evaluated the prevalence and prognostic effect of high tumor mutation burden (TMB) in 10 solid tumor malignancies, among patients not treated with immunotherapy. We found that there is a wide range of TMB among these rarer cancers. Non-inferiority testing and HR estimates comparing OS for TMB-H (≥10 Mut/Mb) vs. -L did not support an effect of TMB on overall survival. We tested the hypothesis that patients with tumors harboring BRCAdel may have prolonged benefit from PARP inhibitors (PARPi) treatment compared to those harboring other BRCA alterations, since tumors with homozygous deletions would be unable to develop acquired resistance to PARPi therapy.

As tumor-agnostic therapies are a new paradigm, it is important to assess their use and effectiveness in routine clinical practice. We examined the use of pembrolizumab in a tumor-agnostic study of 148 microsatellite instability-high (MSI-H) patients across 36 tumor types using a real-world clinico-genomic database.

EGFR exon 19 deletions (x19dels) are well-established targetable drivers in non-small cell lung cancer (NSCLC). Historically, x19dels have been treated as a single group, but it is unclear whether responses vary for distinct subtypes. We used the Flatiron Health - Foundation Medicine clinico-genomic database to compare demographic, clinical and genomic characteristics, as well as outcomes to EGFR tyrosine kinase inhibitors (TKIs) for advanced NSCLC patients with x19dels of varying lengths.

Goal of this study was to explore patient characteristics and outcomes in HER2- / HR+ male, metastatic breast cancer patients using Flatiron's de-identified EHR database. The results of this analysis were used in Pfizer's supplemental New Drug Application (sNDA) to expand the indications for palbociclib into this rare patient population in April 2019. This was the first time that Flatiron data was used as clinical evidence for any approval by the FDA.
