ASTHMA PROGRAM

The Asthma Life Skills Academy for Adults (ALSAA)

This program provides asthma and air quality education, case management services, and resources for adults and seniors in Long Beach. The program is funded by the Port of Long Beach Mitigation Grants Program. 

The Asthma Life Skills Academy for Adults (ALSAA) Project is funded by British Petroleum Settlement Funds and the Air Quality Management District (AQMD). The goals of the ALSAA Project are to improve the health and well-being of adults with asthma and to assist communities in West Long Beach to reduce and/or eliminate indoor and outdoor air pollutants that may contribute to asthma, allergies, and other respiratory illnesses.

In-Home Asthma Case Management Program

An adult asthma education and outreach program for tenants to participate in a one-on-one, in-home asthma care program. The ALSAA Program has adapted the American Lung Association's "Breathe Well, Live Well" curriculum, where adults with asthma learn real hands-on skills to effectively manage their asthma.

                 ALSAA In-Home Asthma Case Management Program

An adult asthma education and outreach program for tenants to participate in a one-on-one in-home asthma care program.  A community health worker will come to your home and educate you on asthma, its triggers, and how to make your home a healthier place to live by using non-toxic cleaning supplies.

Are you or do you know an adult with asthma that would like to participate in the ALSSA tenant outreach education program?  If so, please call (562) 570-4512 to speak to a public health specialist to schedule an appointment with one of our community health workers.

Home Visit One - Recruitment and Enrollment

The Community Health Worker (CHW) either recruits participants through door-to-door visits in local neighborhoods or contacts people that have been referred to us.  If it is a referral, an appointment will be made by phone.  At the initial visit, the CHW begins to fill out the Client Assessment Survey to gather pertinent information on the participant.  The CHW asks permission to do the Asthma Home Environment Checklist, which assesses certain asthma triggers that may exist in the home.  The participant is then asked to complete the American Lung Association's Breathe Well, Live Well (BWLW) Asthma Questionnaire and the Mini Asthma Quality of Life Survey.  As an incentive for participating, the client may be given a bottle of Murphy's Oil Soap, Boric Acid, or Baking Soda.  At the end of the first visit, the CHW schedules the next visit with the participant.

Home Visit Two - Introduction to BWLW and Health Education I

During the second visit, the participant receives their copy of the BWLW an Asthma Management Program for Adults Participant Workbook, which will be used during the second and third visits.  It is the primary health education tool that the participant will receive.  At this visit, the CHW goes over Lessons 1-3.  The CHW also reviews the Asthma Medicines Chart and gives instructions on using peak flow meters and spacers.  As an additional incentive, the participant may receive their choice of a peak flow meter or spacer.

Home Visit Three - Health Education II

At this visit, the CHW completes the lessons in the BWLW An Asthma Management Program for Adults workbook.  The participant also completes the second BWLW Asthma Questionnaire, along with the BWLW Participant Evaluation Form.  Additionally, the participant may receive information on cleaning with non-toxic cleaning supplies to reduce the allergen triggers in their home, along with information on smoking, smoking cessation, and possible referrals to other City departments as appropriate.

Follow-up Visit - Three months post-enrollment

The follow-up visit consists of data gathering three months post-enrollment.  At this time, the Client Assessment Survey Sections D,E, and F are completed.  The CHW asks permission to do the Asthma Home Environment Checklist to determine any changes that may have been made.  The participant is asked to complete the BWLW Asthma Questionaire and the Mini Asthma Quality of Life Survey.  If the participant expresses an interest in reviewing any of the BWLW lessons, the CHW provides a review and answers any questions.  As a thank you for their participation, clients are given a gift card to a local supermarket or discount store.
