Please remember the following regarding Course Code Requests:
- Districts must complete a Course Code Request Form for any new courses, changes to existing courses, or deactivating courses.
- To allow enough time for the MDE to review and consider requests, this form should be submitted by October 1st of the school year before the change will take effect.
