    DETAILS OF INVESTIGATION : As stated

    - That OOA 2250H 23 April 2019 a concerned citizen called at this station informing that
    there was a fire on-going at Sitio Macatoy Brgy. Sta Maria, Calauag, Quezon.
    Immediately thereafter duty BFP personnel responded to the said fire incident. Isuzu
    Automatic Fire Truck driven by FO3 Don Conrad A Macato together with SFO2 Gregorio
    O Anglo Jr when the first responding team arrived at the fire scene and confirmed that the
    fire alarm was positive the second responding team immediately followed, the FMC
    Roughneck Fire Truck (606) driven by SFO3 Menarcito E Revadulla OIC, together with
    FO1 Manuel Louie D Villate proceed to the fire scence. SFO3 Revadulla called
    assistance to Lopez FS immediately thereafter Lopez FS personnel responded headed
    by SFO3 Rio Balilo, OIC Lopez FS and the fire was under control OOA 2300H and put
    out through the joint effort of BFP personnel.
