Availability for 'entitled' Polish citizens abroad

"...issued by the local Office of Civic Affairs. Polish citizens living permanently abroad are entitled, but not required, to have one."

So, abroad Polish citizens may have one, but it is only available at said offices in Poland? How exactly does it work for people abroad? Can they obtain one at the consulate, or can they obtain one when visiting Poland, or requesting through letters?
