Lists name of patient, dates of illness, parents and their address or address of nearest relative. Also includes a list of medications and amounts in the back. Surgeons for the unit include Major T. Jay Robeson, surgeon; Captain Jesse Rowe, assistant surgeon; and Lieutenant Albert E. Mowry, assistant surgeon.

Each is signed by Captain Peter Davidson and countersigned by Quartermaster Sergeant Abraham B. Batterson.

11th Illinois Cavalry, Company D appointment certificates
Certificates of appointment of James E. Kanouse and John H. Kilver as sergeants in Company D of the 11th Illinois Cavalry. The certificates are signed by regimental commander Robert G. Ingersoll. Kanouse's appointment was made in Peoria, Illinois, on 1 March 1862. Kilver's appointment was made in Corinth, Mississippi, on 22 October 1862.
The original certificates are stored in oversize, but copies are available for use.
