We had the pleasure of welcoming the Mayor of the City of Waukegan at Erie HealthReach Waukegan last week! Mayor Taylor took a tour of our health center and met several of our team members, including some of our medical assistants and our dental, social drivers of health, and reproductive health teams. We thank Mayor Taylor for visiting us as we continue to celebrate our 10th anniversary of being in Waukegan and serving more than 10,000 patients! #NHCW24 #ValueCHCs #NHCW

🌱 Health Equity Begins at Home 🏡 In Chicago, where you live can determine how long you live. The life expectancy gap between neighborhoods is a glaring example of how deeply racism impacts our health. The recent op-ed by Dr. David Ansell and Dr. John Rich from Rush University Medical Center sheds light on this critical issue. At Sankofa Wellness Village, we are committed to transforming these inequities by providing holistic, community-centered care that addresses both the symptoms and the root causes of health disparities. Together, we can close the gap and ensure a healthier, longer life for everyone in our communities. Read the full op-ed to learn more about the fight for health equity: https://bit.ly/3XkPY0b #SankofaWellness #HealthEquity #ChicagoHealth #CommunityCare #RacialJustice #LifeExpectancy

At Erie Family Health Centers, we are committed to improving the healthcare experience for our most underserved communities. As an owner and participant in the Medical Home Network Accountable Care Organization (MHN ACO), we are working in collaboration with other healthcare providers and the CountyCare Medicaid health plan to drive innovation, improve care, and enhance health equity for the people of Cook County. Our Chief Strategy and Transformation Officer, Nicole Kazee, recently testified in support of CountyCare as a critical component of the Cook County Health budget. The MHN ACO brings together 13 Federally Qualified Health Centers, three hospital systems, and their physician practices with one goal: to transform healthcare delivery for Cook County’s most economically and socially marginalized communities. #HealthcareTransformation #HealthEquity #Innovation #CareManagement #CommunityHealth #ACO #CountyCare #CookCountyHealth

We loved being a part of the Evanston Back to School Fun Fest and resource fair, which served an estimated 600-700 families! In addition to providing free food, kid-friendly activities, give-aways, and resources for parents, the event focused on five protective factors for strengthening families: resilience, relationships, knowledge, concrete support, and communication. YJC joined Erie Family Health Centers Evanston/Skokie, Open Communities, and Youth & Opportunity United in representing the "knowledge" section of that tapestry. Thanks to Be Strong Families and City of Evanston for sponsoring, to Robert Crown Center for hosting, to all the wonderful organizations spreading the word about their community offerings, and to everyone who attended and made it such a fun time! Thanks also to Evanston RoundTable for a great write-up, which we recommend as a short and informative read about what the Back to School Fun Fest has to offer for local kids, parents, and families. (It's scheduled for every August—we hope to see you there next year!) https://lnkd.in/g7iMew4h

On Wednesday, YJC staff showed up for the third annual Evanston Collective Summit, where ten youth-serving nonprofits explored concrete ways to embody our community standards. YJC executive director Cree Jamison spoke to the group about just what those community standards mean and why they’re important in our work with young people and their families. It was an edifying day full of meaningful conversations, inter-agency connections, and new ideas for providing excellent care to the people we serve. Thank you to all our fellow Collective members—City of Evanston's Youth & Young Adult Division, Connections for the Homeless, Curt's Café, Erie Family Health Centers, Evanston Cradle to Career, Infant Welfare Society of Evanston, James B. Moran Center for Youth Advocacy, PEER Services, Inc., and Youth & Opportunity United—for your continued collaboration, and special thanks to James B. Moran Center for Youth Advocacy for organizing it all!
