​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​The Nebraska Chronic Disease Prevention &​ Control Program (CDPCP) utilizes federal funding to collaborate with federal agencies, state programs, and community partners to improve the prevention and control of heart disease, stroke, diabetes, and cancer along with associated risk factors including: hype​​rtension, high cholesterol, pre-diabetes, overweight/obesity, physical inactivity, and unhealthy eating.​ ​​

Current federal funding for the CDPCP include: ​

• The National Cardiovascular Health Program​
• A Strategic Approach to Advanc​i​ng​ Health Equity for Priority Populations with or at Risk for Diabetes
• 2021 Empowering Communities to Address Behavioral Health and Chronic Pain through Chronic Disease Self-Management Education Programs​
• ​Cancer Prevention and Control Programs ​for State, Territorial and Tribal Organizations​​
