Meet our Federally Qualified Health Center partners! First up is AppHealthCare, that serves residents in northwestern North Carolina! "The Appalachian District Health Department was established in 1933 and covers Alleghany, Ashe and Watauga Counties. The health department provides many services such as high quality clinical care, nutrition services, Women Infants and Children (WIC), community health services and environmental health services. Its mission is to promote safe and healthy living, prevent disease, and protect the environment." What was one of the highlights our team took away from the 2024 National Association of Community Health Centers (NACHC) CHI & Expo Conference in Atlanta? The knowledge we gained listening to the presentation made by our teammate, Kati Moran, and Lisa Jones and Anna Marini from the Duffy Health Center, and Michele "Chrissy" Kuahine, from the Waianae Coast Comprehensive Health Center, which explored how community health centers can develop culturally competent digital health employees to meet the diverse needs of their patients. The work being done in this vein is vitally important, as health center leaders craft solutions to enhance patient-centered care in the ever-evolving digital landscape. How can community health centers enhance their high-risk maternity care programs with data-driven strategies? Working in partnership with our Arcadia colleagues we: - Connected our electronic health record (EHR) data to Arcadia's platform. - Scheduled annual Social Determinants of Health (SDoH) screenings for our members to determine what their needs were. - Used the information we collected for care management assessments. The data allowed us to effectively identify high-risk members and provide them with the care they needed. "Lou has served in the Federally Qualified Health Center movement for two decades, beginning as Chief Operating Officer for Lowell Community Health Center in Lowell, MA. Lou also served at CHCI in Meriden, CT as Site Director; at Geiger Gibson CHC at Columbia Point in Boston, MA as Executive Director; and the Cornell Scott Hill Health Center in New Haven Connecticut at Chief Operations Officer. Lou knows that the while we must constantly adapt to meet the demands of a changing healthcare landscape, we must never compromise in our commitment to our patients, staff, and community. Lou holds a Bachelor of Science degree in Industrial Management from the University of Massachusetts at Lowell, and a Master's in Business Administration from Northeastern University." And the skills, experiences, and “voices” brought to our Board by these individuals allows us to produce the medical programs and care plans to deliver great health to all of our members. Say hello to Elizabeth Browne, MBA, Chief Executive Officer at Charles River Community Health! "Elizabeth was appointed Executive Director of Charles River Community Health in 2009, and as the organization has successfully grown and restructured became its Chief Executive Officer in 2019. Liz has been with CRCH since 2003 and served previously in senior leadership positions in operations, development, and strategic growth. She has over 25 years of experience in health care technology, operations, managed care network development and technical assistance, and policy analysis and development. Liz is the Chair of the Massachusetts League of Community Health Centers Board of Directors, and also serves as Chair of the Community Care Alliance Board of Managers at Beth Israel Lahey Health. She received the Executive Director of the Year award by the Massachusetts League of Community Health Centers in 2018. Liz holds a Masters of Business Administration in Health Sector Management and Bachelor of Science in Biomedical Engineering, both from Boston University."
