✍🏻 Connections Blog: Social Care Networks Announced Under New York’s HRSN Waiver ✍🏻 Yesterday we shared the news that Findhelp has been selected to partner with several Social Care Networks (SCNs) as part of New York's 1115 Medicaid waiver. Today, we're sharing more information and highlighting one of our SCN partners, SOMOS Community Care 🗽 SOMOS is a network of over 2,600 culturally competent healthcare professionals who provide care to over 1 million patients in New York’s underserved neighborhoods—particularly Asian, African-American and Latino communities. Their network comprises primary care physicians, specialists, behavioral health providers, pharmacies, community-based organizations. As the designated SCN covering the Bronx for New York’s 1115 HRSN waiver, SOMOS will partner with Findhelp to further empower their CBO partners, create additional capacity around social needs screening, and deliver the state’s enhanced HRSN services to eligible populations. We're honored to partner with several SCNs via our technology that enables eligibility determination, health-related social needs screening, closed-loop referrals, and social care payment and claims submission. We look forward to working closely with the New York eHealth Collaborative's Statewide Health Information Network of New York (SHIN-NY) and the Qualified Entities to support interoperability and standardized social care data exchange.
