Join Us on Today at 4 pm ET: Current Controversies in Critical Care Webinar! The field of critical care medicine continues to evolve, with new evidence constantly challenging the existing beliefs. While analgesia-first sedation strategy avoids the use of sedative drugs, the use of #opioids to achieve this may lead to acute withdrawal in the short term and persistent opioid use in the long term. Similarly, the timing and choice of vasopressor administration in patients with septic shock remains unclear despite extensive #research, and whether renin as a biomarker should replace lactate in #sepsis treatment is being argued. Post-intensive care syndrome (#PICS) continues to challenge critical care providers, and the role of PICS clinics in improving outcomes is unclear. 🎉 Congratulations to my colleague and fellow #PINNACLEPeriop researcher, Bijan Teja, on being awarded a 2024 IARS Mentored Research Award (#IMRA) by the International Anesthesia Research Society! 🎉 Bijan is an #anesthesiologist, #criticalcare physician, and #earlycareeresearcher at Unity Health Toronto and Temerty Faculty of Medicine, University of Toronto. His groundbreaking research on the potential role of fludrocortisone - a mineralocorticoid with unique anti-inflammatory properties- in critically ill patients has already resulted in high-impact publications in JAMA Internal Medicine (https://lnkd.in/g8yKXeDD) and American Journal of Respiratory and Critical Care Medicine (https://lnkd.in/gAD_wTJh). This new grant will propel his program of research forward, building towards a large multicenter randomized trial of this novel intervention aimed at improving outcomes for the 90 million adults globally who suffer from acute hypoxemic #respiratoryfailure each year. I am grateful to the International Anesthesia Research Society for funding this critical work and for its commitment to advancing #earlycareeresearcher careers in #anesthesiology. Our research paper is now published ahead-of-print! 🎉 Obesity is highly prevalent in adults living in North American countries (30-40%) and patients with obesity undergoing surgery are at increased risk of pulmonary complications after surgery, including the collapse of lung alveoli (known as atelectasis). However, little is known about atelectasis being already present before surgery. In our study published in the International Anesthesia Research Society journal Anesthesia & Analgesia, we show that the prevalence of atelectasis before bariatric surgery is high (32%) and markedly increases at higher BMI values. Through a biological mediation analysis, we show that atelectasis are not merely postural (chest CT is taken in the supine position), but mediate ~80% of the effect of increasing BMI on low oxygen saturation levels before surgery in a different position (patient seated).
