A recent article in Healio’s Nephrology News & Issues highlights the important role advanced practice providers play in caring for people with #kidneydisease. With our interdisciplinary approach to value-based kidney care, our nephrology partners have access to support including advanced practice provider (APP) education and onboarding to help them provide the best possible care for their patients. https://lnkd.in/e82HEb_V

A national survey of patients, care partners, and providers found lack of access to comprehensive patient education is a significant barrier to increasing rates of home #dialysis. Interwell helps our provider partners overcome barriers to home dialysis through a value-based kidney care model that prioritizes patient education through our robust education platform, Interwell Learning. Our comprehensive, interdisciplinary approach to value-based #KidneyCare includes addressing the social determinants of health that impact our patients’ health and wellness.     By using SDoH data such as housing instability, transportation needs, and food insecurity, we can pinpoint at-risk individuals and empower them to get the care and resources they need.
