Working in partnership with health care providers and community organizations, UCare offers: • Medicare Advantage plans across Minnesota and parts of Wisconsin • Minnesota Senior Health Options (MSHO) for people age 65 and older who have both Medicare and Medical Assistance • Institutional Special Needs Plans (I-SNPs) • Individual and family health plans, including plans with M Health Fairview and North Memorial Health, available through MNsure • Minnesota Senior Care Plus (MSC+) for people age 65 and older on Medical Assistance • UCare Connect plans for adults with disabilities (physical, development, mental) • UCare Connect + Medicare for adults with disabilities who have both Medicare and Medical Assistance • EssentiaCare Medicare Advantage partnership plans with Essentia Health in northern Minnesota and parts of Wisconsin • M Health Fairview & North Memorial Medicare Advantage partnership plans in the metro • MinnesotaCare and Prepaid Medical Assistance Program – Income-based programs for individuals and families We're located in Northeast Minneapolis near major freeways, public transportation sites and convenient retail centers. 🏆 Teamwork + community = UCare #peoplepower Last weekend, the Gus Macker Basketball Tournament took over the Twin Cities. This 3-on-3 event creates a vibrant hub of competition, camaraderie, and family fun for all ages and abilities. UCare's Christen Glass, Commercial Team Manager in Government Relations, and his son brought their love of sport and community to the games. Christen reported "my son's team - Tune Squad - placed 3rd. They were all 8 or younger playing in a 10 and under bracket." PS. Christen's team won the toilet bowl trophy.
