September is Sickle Cell Awareness Month, and at Healthmap Solutions, we want to call out the link between sickle cell disease (SCD) and chronic kidney disease (CKD). Healthmap Care Navigators educate our program members on the advantages of in-home dialysis over in-center treatment, including greater convenience, flexibility, and comfort, all of which can lead to better clinical outcomes. Among members in Healthmap’s Kidney Population Health Management program, 20% to 40% of new dialysis starts utilize home dialysis modalities. That’s substantially higher than the national average of 13.3% reported by the United States Renal Data System!
