Services Offered

The Integrated Health Medical Home services Soldiers and their families who are enrolled in TRICARE for their primary care needs. Patients are seen by appointment only with limited same-day appointments for acute medical problems. To best service you, patients are requested to arrive at least 15 minutes prior to the scheduled appointment time so that screening can be done prior to seeing your provider. 

About Us

The Integrated Health Medical Home provides comprehensive, patient-centered care for our military community that aim to improve readiness and promotes health for our military members and their families. We provide care that puts you at the center of a team of health professionals working together to improve your health. Your Medical Doctor, Physician Assistant or Nurse Practitioner will lead a team of healthcare experts in managing your healthcare needs.

In addition to you, the team may also include:

• Nurses, Medical Assistants, and Health Care Specialists working with your provider before, during and after your visits
• Nurse Case Managers
• Clinical Pharmacists
• Respiratory Therapists
• Health Educators
• Behavioral Health Consultants

Clinic Hours

Monday-Friday 0800-1600
Every 3^rd Thursday afternoon from 1200-1630
Closed: Weekends and Federal Holidays
Moncrief does not have an Emergency Room. In the event of a true emergency, please proceed to the nearest Emergency Room or call 911.
 

Walk-in Treatments

In addition to clinic hours, please note the following times for walk-in treatments
Treatment Hours: Monday-Friday between 0800-1130 and 1300-1530
Closed: Lunch 1200- 1300
Walk- In treatments are treatments prescribed by a health care provider prior to service and can be completed on walk-in basis without the need for an appointment.

• Medication Injections
• Pregnancy Tests (no prescription needed)
• Blood Pressure Checks
• Other provider prescribed treatments as appropriate

** Please note: Wait times may vary and may be prolonged for walk-in services. This wait may depend upon available staffing and the number of patrons waiting to be serviced. Patrons will be serviced in the order that they arrive. Please plan accordingly. We appreciate your patience and understanding.

Forms Requests

Forms Requests: When requesting forms to be completed by your health care provider, please allow 3 business days from the date of submission for forms to be completed. Forms will be available for pick-up after this timeframe.

IHMH Clinic Patients
Many issues addressed in primary care have a behavioral/psychological component.  Learning new habits and behavioral techniques can help address these issues.

Are you having difficulty with any of the following problems or concerns?

• Insomnia                  
• Weight management          
• Depression/Anxiety
• Chronic pain
• High blood pressure                       
• Stress management           
• Sticking to an exercise plan          
• Quitting tobacco
• Alcohol problems
• Any other health issue that may have a behavioral/habit component

If so, ask your PCM about including Behavioral Health Consultation as part of your treatment program.
Behavioral Health Consultants (BHCs) work closely with your PCM and provide recommendations to your medical providers and help you to develop skills, change habits, and use available resources to better manage problems affecting your health and well-being.

Patients can also self-refer by calling 751-2168 to schedule with the Behavioral Health Consultant.
