District Court Referee Forms

For all court hearings, the parties are expected to be courteous and respectful to the Court and to the other parties present in the Courtroom. This means you should remove any hat or head covering not worn for religious or medical reasons and turn your cellular phones off.

Exhibits that you wish to bring as evidence must be submitted to the District Court Referee at least two (2) business days before your court hearing. Exhibits should be emailed to This email address is being protected from spambots. You need JavaScript enabled to view it., with each exhibit being a separate pdf. If exhibits are sent as one single file, the exhibits will be rejected and must either be resubmitted or submitted in paper format. If you choose to bring paper exhibits, you will be charged a fee pursuant to the Nebraska Supreme Court to transfer all paper exhibits to a pdf should you decide to file an exception (or appeal) from the District Court Referee. Paper exhibits must be single-sided. Below is a list of items you may want the Referee to consider at your child support hearing:

  1. When establishing support:
    1. Bring photo ID (you will want to have this with you in person, not submitted as an exhibit);
    2. Proof of your income (Recent paystub, W-2, tax returns, profit/loss statements if self-employed, etc);
    3. If you have health insurance (not Medicaid) for yourself and/or your child/ren, you may want to submit:
      1. Proof of carrying health insurance (i.e. a recent pay stub showing the deduction, confirmation of enrollment page, etc.)
      2. Proof as to the premium cost for you alone; and
      3. Proof as to the premium costs for adding the child/ren to your insurance policy;
    4. Information as to any other child support orders you are ordered to pay;
    5. Information that demonstrates your contribution to a retirement plan if you have one;
    6. Information (including birth certificates or adoption documents) for any other children for whom you provide support outside of a court order;
    7. Your proposed child support calculation if you wish to present your own to the Referee. See the Forms section for the site where you may prepare your own calculation.

  2. When Modifying Support:
    1. Bring photo ID (you will want to have this with you in person, not submitted as an exhibit);
    2. Last three years' tax returns and proof of current income (Recent paystub, W-2, tax returns, profit/loss statements if self-employed, etc);
    3. If you have health insurance (not Medicaid) for yourself and/or your child/ren, you may want to submit:
      1. Proof of carrying health insurance (i.e. a recent pay stub showing the deduction, confirmation of enrollment page, etc.)
      2. Proof as to the premium cost for you alone; and
      3. Proof as to the premium costs for adding the child/ren to your insurance policy;
    4. Information as to any other child support orders you are ordered to pay;

  3. For Contempt (Show Cause) Proceedings:
    1. Bring photo ID (you will want to have this with you in person, not submitted as an exhibit);
    2. Proof of your current income (Recent paystub, W-2, tax returns, profit/loss statements if self-employed, etc);
    3. Proof of any recent child support payments you may have made;
    4. Proof of any disability (doctor’s notes, medical records, etc.) you claim is the reason for your inability to make payments.
